Step 1

Mental Health Assessment

Participants who are justice-impacted can securely complete standardized Evidenced-Based mental health assessments such as the DASS-21 (Depression, Anxiety, and Stress Scale), ACES (Adverse Childhood Experiences) Questionnaire, and the SWLS (Satisfaction With Life Scale) through a HIPAA-compliant online platform. Through our platform, participants receive a secure link via email, text, and portal access to complete the assessments on a mobile device, tablet, or computer. The system automatically scores the responses and generates a clinician-ready summary, organized for easy interpretation while maintaining data integrity and compliance with privacy regulations. These digital tools can also track progress over time and store results in alignment with HIPAA security standards. After participants finish the online assessments, participants will meet with one of our licensed clinical therapist's who reviews the data and completes a comprehensive mental health evaluation. During this virtual meeting —the therapist integrates the assessment findings with the clinical interview to finalize a diagnostic impression and establishes a care plan collaboratively with the participant. This process streamlines intake, protects participant privacy, and supports consistent, evidence-based evaluation for individuals transitioning through or impacted by the justice system.

Group Therapy and Brief Individual Therapy

  • Reentry is not just about leaving incarceration—it’s about healing, rebuilding, and staying connected. Our telehealth process groups, brief individual therapy sessions, and counseling groups offer justice impacted individuals a safe, accessible space to explore emotions, build coping skills, and develop the tools needed for lasting success beyond confinement. Brief individual therapy provides personalized support focused on immediate challenges, emotional regulation, goal setting, and problem-solving to help participants navigate barriers during the reentry process. Recidivism—the tendency to relapse into criminal behavior—can stem from trauma, untreated mental health struggles, limited social support, and structural barriers. By focusing on emotional processing, practical skill development, and social connection, our telehealth services cultivate trauma-informed care and foster the resilience and self-understanding needed to prevent reoffending. Participants learn how to manage conflict, rebuild trust, strengthen decision-making skills, and envision a future rooted in purpose. Telehealth counseling transforms isolation into connection. Through both group support and individualized therapeutic interventions, participants are empowered to take ownership of their lives, relationships, and emotional wellbeing—key factors in breaking the cycle of incarceration. Together, we’re building communities where every person has the support to heal, grow, and stay free.

Essential Supportive Services

Radical Transformation Inc. is committed to meeting the immediate and foundational needs of justice‑impacted individuals as they work toward stability, healing, and long‑term success. To reduce barriers and promote overall well‑being, the agency provides the following essential support services: 1. Community‑Based Food Assistance Program This program assists with access to nutritious food for participants experiencing food insecurity. By partnering with local community organizations, Radical Transformation Inc. ensures individuals receive immediate relief, helping them stay focused on rebuilding their lives and pursuing personal, educational, and employment goals. 2. Care Packages & Hygiene Products Participants can receive essential hygiene items that support dignity, health, and daily functioning. Care packages may include toothbrushes, toothpaste, soap, shampoo, deodorant, and feminine hygiene products. These items help individuals meet basic needs that are often difficult to access during reentry, reinforcing confidence and supporting successful reintegration.

Desired Outcome #1

Participants will not be arrested or reincarcerated, convicted of a new crime, or have their supervision revoked once they become a Participant at Radical Transformation Inc.

Desired Outcome #2

To improve the Quality of Life of participants by 25-66% or greater from baseline on the Satisfaction with Life Scale (SWLS). 100% increase being the Best Positive Outcome.
